Adding rates to card

I am trying to add a rate to a card but the rates page is a bit confusing.

I see destination, prefix and rate.

What is the destination value?

I have more than 10 prefixes for a single network, do I add all the in one go or add each independently?

Let me give you a background of how our charging works.

Depending on how much someone pays, they automatically assigned a tier e.g.

less than $100 rate is 0.8
> 100 and less than 500 rate is 0.6
> 500 rate is 0.4

How can I capture that in the rate page?
